Please look at pdf. My focus was to provide a clean and clear design that is easily expandable through both print and web. Since the many of the sections can be varied in size, I made sure to provide a layout that would be easily printed out no matter the amount of information. I divided the Health Record in to 3 section; General Medical Overview, Medical History, and Lab results. After looking and the example provided I saw that all of the Medical history fields (Plan of Care, Encounters, and Problem List) could all be grouped together and centered around a health problem. This way the patient has a simple way fo understanding what went on over the years.
