In this submission, I have tried to limit the amount of interpretation and have instead focused on practicality and reporting. In this effort, I've been true to the format of a doctor's note (leaving ample space for categories such as History of Present Visit, Past Medical History, and so on), but have kept simple design principles.

Additionally, I have designed the deliverable to be a print-out that a patient actually takes after a hospital appointment (the most common interaction for a patient with their doctor). To showcase this affect, I have labeled all the pages appropriately so that even if out of order (as my submission demonstrates), a patient would be able to follow the page numbers and piece together their medical record.
