I designed this submission to be incredibly efficient for a physician or other health care provider to read and get relevant data from. I also designed this submission to be readable by the patient; there are areas within the EMR that are designated to provide information and descriptions to the patient. They color coded in green and labeled with a big header: “Comments to the Patient.” In order for this to be feasible in a real world health care practice, the patient information comments would have to be largely computer generated, with the physician being able to edit or add to these patient comments.

With the submission, I have tried to create a document with the best of both worlds: efficient and accessible information for the health care providers, as well as an English interpretation for the patient.
