I wanted to create a profile with the picture of the patient on each page to avoid pages being mixed with other patient's profile.The purpose of having the picture on the first page would also avoid the confusion of patients with the same name. I designed a profile with the picture of the patient front and side view that way doctors could identify the patient easier when it comes to an emergency. In addition the first page includes the most important information, for example allergies, type of blood , illness, ages , emergency contact, weight and more. The other pages complement this information. The simplicity of the design of this format would allowed it to be access and applied to mobile applications and websites.
