Built on Twitter Bootstrap, this theme will adapt to various screen sizes, including tablets and other mobile devices. It also has large type and the sections are numbered to make them easy to identify (over the phone, for example).

My wife is as a clinical social worker and provided valuable feedback for ordering the record and suggested a few additional pieces of information (e.g. last prescription refill, NIH links for conditions/problems).

There are still a bunch of refinements I'd like to make, but this is a good start.
