Patient data is grouped label-style across the first half of page one. Problem list, allergies, medication and immunizations come next to complete the current state picture.
Encounters and procedures are combined because the difference is not meaningful to patients. Chronological views of past and future treatment plans are displayed in columns to avoid repetition of data headings.
I'm not submitting a lab sample because anything I do can come up with isn't as good as what Goetz presented in the TED talk "It's time to redesign medical data".
