For this submission I was inspired by the original "receipt" feeling and decided to expand upon it with color bars and an easy-to-read format using color and font hierarchy. The addition of simple graphs indicating severity or where one falls within range of lab results makes this layout creative, interesting, and best of all: easy to read and follow.
