A Health Design Challenge submission focused on simplicity and practicality. This design allows for easy integration – for both print and web – as well as provides opportunity for expansion.
By methodical grouping of key information, this design provides flexibility for simple to complex medical history, with ease of use for the Patient, Caregiver(s) and Provider(s). These are some of the many use cases taken into consideration during design:
• The Patient Use Case: Patient is an elderly person who has a somewhat complex medical history and requires a number of follow-ups and medication. After meeting with the doctor, Patient has difficulty recalling what was discussed and has a medication schedule that requires multiple pills, multiple times a day. Having something clear, simple and easy to follow is important for this Patient.
• The Caregiver Use Case: Caregiver was unable to attend the session between the Patient and Doctor, but is responsible for Patient’s medication and follow-ups. After the Patient returns from the visit, the Caregiver can review what the Patient priorities are and help the Patient with their medication regiment.
• The Provider Use Case: Patient/Caregiver may have additional follow-up questions or require an opinion from a different Provider. At the appointment, Patient brings in a print-out of their medical history downloaded. The Provider may use this designed print-out as a discussion guide with the Patient as it has enough preliminary information – such as history of visits, procedures and lab results - to begin conversations or point out trends. Additionally, Patients are known to take in a medication sheet with their medication list and handwritten notes to show Providers. With this, it can also expedite the check-in process with the Nurse by providing answers to the most frequently asked questions during a Patient’s visit.
The minimalistic design keeps in mind a Patient's viewing ease, while retaining convenient printing ability.
