My Health Time-Line: What's Next and What's Relevant?
This design focuses on effectively addressing the two most common patient questions: (1) What should I do next? and (2) What has happened in the past that is relevant to my situation? Conditions, treatments, and events are organized on parallel tracks along a vertical time-line with the future at the top and the past at the bottom. Self-explanatory icons and meaningful labels provide an overall picture, with additional information available via a single click on each element. Search and selection tools allow the user to focus on specific content. Identifying information is prominently presented on a top line. Information less-directly relevant to the two questions (e.g., insurance) is available via a single click on a set of expanding tabs above the time-line.

Very interesting design. I like the vertical timeline and the appearance of a simple way to expand/narrow the scope of the timeline.

Designing the interaction for the non-linear time line was probably the toughest part of the design. We decided to go for a well-understood interaction model based on opening and closing hierarchical trees. It also combines nicely with an "auto-expand" feature that allow the user to zoom in on any part of the time-line that contains data by clicking on any ellipsis, which serves as a hidden-data indicator.
