Universal Patient Health Record
The idea behind this record was to organize the extensive data behind an individual’s medical history into a template that would be both easy to follow and easy to maintain. It was also an objective to create a file that could be equally effective either as a physical paper document or as an online form. The data is organized into sections so that the needed information can be quickly located. The particular sections are also formatted in a way that additional pages to document more information can be added with disrupting the entire file. It was purposely kept simple as to increase usability for people of varying cognitive abilities. Metadata was kept to a minimum to increase electronic searchability and accuracy. In this manner, exhaustive patient information can be stored without significantly increasing the search cost to doctors, medical professionals, or the patient themselves. Lab tests were designed around a standard form that could be supplied to medical laboratories for standardization purposes. Each lab report is accompanied by any alerts that may have been triggered by the results, as well as suggestions to improve the health of the patient. A premium was put on creating a file that would not only be useful now, but would have the potential to capture any pertinent future information, effectively creating a system that would increase in value as it was used.
