CarePASS - Care Plan Automated SnapShot
CarePASS is a print version of a patient's personal health record and care plan, which aims to display a large amount of patient data in a compact, yet simple display that is usable and meaningful to both the patient and clinician.
I avoided complex graphics or icons because I recognize printer quality may differ among users. Also, I utilized color in CarePASS but ensured the contrast in colors still provided the intended meaning in grayscale. My intent was to minimize redundant 'data ink' by using contrasting font styles and sizes.
CarePASS could easily be adapted into an interactive, dynamic display of patient information. Due to time constraints, I focused on this static display but would include timeline-based displays of certain sections of patient information that I believe would provide additional insight for both the patient and clinician.
Today, the healthcare system is collecting thousands upon thousands of data points on each patient and we need to focus on providing meaningful displays of this data, like CarePASS, to both empower patients and inform clinicians. This effort will ensure this ever-expanding repository of data can be utilized to improve the care process for each patient.

I've just been browsing through the submissions and this one stands out (for me) so far because it's got a good high density of information.
Most submissions I've seen so far seem to spread everything out with a lot of additional detail, I guess under the premise of making things easier to understand.
But I don't think that's the right solution to help people with low medical literacy. Anyway, nice work on the submission!
