The personal health record was redesigned by keeping in mind the patients' need for understanding the complex information in an easy and effective way. The design serves well not only for patients but is also a quick summary and reference for their physicians, which can be extremely critical in emergency situations. According to the Department of Health and Human Services (HHS), 20% of preventible medical errors are caused by the lack of immediate access to health information. We designed the PHR that is equally beneficial to the physicians, caregivers and the patient himself.

(The design focuses on providing a better solutions to the existing problems. It was originally designed as a printable version of the PHR. However, we also keep in mind current available technology so that the design can easily be converted to HTML/CSS/JS)
