My design is a document that the patient can download and print out from home, via a medical portal of some sort, and can use to educate him or herself on the results of tests, doctor's recommendations, comparisons to national averages, etc. My hope is that this document would be as user-friendly as can be and be written in a more conversational tone than we are used to in medical documents.

Photos are just placeholders and I do not own the rights to use them for commercial purposes. They are just for placeholders. All other artwork was created by myself including icons and charts. I chose the typeface, Delicious, which I do own the rights to and chose it for it's modern, approachable appeal.
