The Personal Medical Record is a visually compelling, easy-to-read medical report that facilitates doctor/client interaction and personal health empowerment, presented as a dynamically built, fully modular, interactive PDF report.

This report would allow medical professionals to present and review data with their patients in an easy-to-read format. Simple icons and color coding—with visual indicators for those with color blindness—give quick reference to important medical information and lab results. They would have immediate access to the names and contact information for previous medical professionals and healthcare facilities for quick reference and consultation.

It would give patients a solid documentation of their current health status which they could keep for review and later reference. It would create a standard format for report creation that would allow for ease in reading and review, and quick location of pertinent data, including immunization records, health recommendations, upcoming appointments, and personalized notes from their medical professional.

The report would be built on-demand upon a modular UI framework, atop the medical records CMS/EMR system. The report is fully tailorable on the fly, including language translation support, allowing medical professionals to have complete control over the presentation and breadth of information included. All page content would be built out modularly, allowing for quick construction of the document, with no more time than needed to navigate a standard print dialog box. The report could be printed for immediate review and handoff to the patient, or could be delivered via electronically to the patient's email address.
