MediRec
The intended audience for the www.MediRec.org web site is both Personal Health Record ( PHR ) owners, the individuals referred to by the PHRs' information, and medical personnel that would use the PHR data when they treat patients. So an efficient PHR should strike a balance between the information entry and display needs of both of these groups.
The www.medirec.org site is a fairly comprehensive design, based on the idea that the PHR owners' won't know much if any medical terminology. So they will need to be guided through multiple relevant choices, but with the option of making their own free-form entries.
However, the PHR is also designed to be maintained by medical personnel, whenever a patient visit occurs. So it must allow entry of sophisticated medical terms when appropriate, without burdening the medical personnel with undue complications in entering information.
The MediRec site at www.medirec.org is a partially-functional mockup of a PHR web site. All the main tabs and submenus allow selection of items, and other features are also functional, in order to demonstrate certain capabilities.
Although information can be entered in some parts of the site, as it doesn't currently have a database implemented, data cannot be saved. But the accompanying www.medirec.us web site is a less comprehensive but fully functional earlier version of a PHR web site, with far fewer tabs and fields, but with a database. And it does have some real-world users.
The final look and feel of MediRec could be anything that's desired. For example it could fit in with a particular corporate look-and-feel, or the design policies of a medical organization such as the Department of Health and Human Services ( HHS ).
The current appearance is based on the American Health Information Management Association ( AHIMA ) set of hard-copy PHR forms ( with their permission ). And these could be modified to provide new functionality, or have a different appearance, if desired.
If any of the explanations of functionality in the accompanying .pdf file are unclear, please take a look at the site at www.medirec.org for clarification, as the site operation is fairly self-explanatory.
