Record field questions (and controlled vocabs)

I'm hoping someone more familiar with the CCD data structure can help me puzzle out how some of the patient fields work.

Looking at the Record PDF & sample data (http://blue-button.github.com/challenge/files/sample-challenge-record.txt) and the CIM core data elements wiki (http://wiki.siframework.org/ToC+CIM+Core+Data+Elements), I'm none the wiser as to which fields in the record belong to a controlled vocab.

Specifically:
Allergy Severity: the sample has Moderate to severe, Moderate, Severe. Is this a controlled vocab? Where can I find the rest of it?
Medication dose: are dose, rate, and type from a controlled vocab?

Other questions:
Lab reports: the sample record has no examples; the Records PDF just has the test results.
Should Test results have a "date ordered", a "Provider", and a "testing center" or similar (like the VA record?)

And I'm sure I'll have other questions as I go along.

    Hi James,

    There is a controlled vocabulary. Much of it pulls from codes from:

    - SNOMED
    - RxNorm
    - LOINC
    - CPT
    - ICDN

    Those are the common libraries that describe the fields.

    The example lab results will be uploaded tomorrow. As will example designs from some of our curators. For example, you can see one here:

    Examples of lipids have been added to:
    http://blue-button.github.com/challenge/files/more-fields.txt

    For the other lab results, there are plenty of resources online that can provide the target ranges and what the lab results look like.

    Thanks for getting us the extra examples Ryan!

    Would it be possible to know what the "Dose" and "Rate" fields look like for a medication that requires continuous infusion? e.g. Morphine IV?
