Are the fields intended to be input?

On the form, do there need to be checkboxes or blank lines for the patient to input information? Or is this expected to be a consumption-only printout.

    Good question - I read a really helpful post in the discussion comparing this to a bank statement - I.e something you could get online but also print out and share with someone. In this analogy, the bank automatically fills in the cells for you - Ryan, is this the same in this instance? The patient could see their summary or specifics but also print it out to share?

    I am a participant, not a moderator but I can tell you what information has been provided in answers to other discussions, which I also recommend everyone read through to see if their questions have already been answered.

    There is no interaction with the data, hence no checkboxes or blank lines for inputting data are needed. Please see the discussion titled "Required fields" where this was answered.

    The main focus is for this information to be something that can be provided as a document to a patient, who may then choose to print it and/or keep an electronic copy of it.

    Ryan (the moderator who has answered other questions) can correct me if I've provided any misinformation.
